Please use one of these forms when requesting to use the church building. Please understand that until you hear back from the office your event is NOT scheduled.

Room Use

This form would be used for a church event such as a game night or cookout. This form can be used for events that happen one time or that are recurring.

Wedding - Member

Use this form if you are a member requesting to use the building for your wedding

Wedding - Non Member

Use this form if you are not a member and would like to use our building for your wedding

Room Use, Non Member - multiple

Use this form if you have a reoccurring event such as a 10 week bible study class, or a monthly meeting.

Room use, Non Member - single

Use this form for a one time event

Family Use - Member

This form is used if you are a member and would like to hold a baby shower, a family reunion etc.. With this form the custodian does not come and clean after your event, you are expected to do all the cleaning.

Family Use - Member with Custodian clean up

Use this form if you are a member and would like for our custodian to clean the building when you are finished.

Recital Form

Use this form if you are wishing to host a recital at the church
